Note: We receive many anguished letters for help, but many times people are so angry and have been involved with the case for so long that they forget to tell us the whole story, from the beginning. But it is very important to lay out the facts, in order. This form is intended to help you do that. provide us with as much information as you can.
Most of the boxes in this form will expand to accomodate as much information as you wish to give us.

1. Narrative. This part is optional. Answering questions 2 through 17 should give us the information we need. But many people like to begin by just telling the story, as if they were telling a concerned stranger what happened. If you provid this narrative, there is no need to be formal or legalistic. 6a. Accused's background. has the accused ever been accused of any other illegal sexual activity (rape of an adult or child? child molestation?) or of any other kind of assault against an adult or child? Please give details about the charges and describe how they were resolbed (dropped? acquittal? conviction? appeal?) has the accused ever been harged with or convicted of a non-sexual non-violent crime, such as possessing or selling drugs?
Compete candor is appreciated in answering this section. We cannot effectively help if you withhold information you might consider embarassing.
6b. Accuser's background. Do you know of any other instances in which the accuser made similar complaints? What happened in these cases?
7. Tell us of any background information about the accuded and accusers' situations or relationship that preceded the accusation and are relevant to it.
8. What happened during the first few days after the allegations arose? Specify the names of the police officers, child protection service workers, and others who became involved. List contact information if you have it.
9. Continue to tell the story of what happened in order of when it happened. Take your time and give as many details as possible. Who is the prosecuting attorney? Describe him or her, and give contact information.
10. What defense lawyers have been involved in the case and when were they hired? Please give contact information for them.
11. Are there expert witnesses or other witnesses involved in the case? Please give contact information.

13. What are the principal reasons you believe in the innocence of the accused?
